Immunization Exclusion Day - February 17, 2016
Immunization Exclusion Day - Wednesday February 17, 2016
Immunizations are required by state law for children and students in attendance at public and private schools, childcare facilities and Head Start programs in Oregon.
If you have not provided the school with the state required immunization documents, or if you have not completed the new forms for Non-Medical exemption (formerly religious exemption), you will have received an exclusion notice.  This is a letter stating that your children must be immunized or will not be admitted to school starting on Exclusion Day, February 17, 2016.
